Geriatric Care Management & Tranansition Service
Geriatric Care Management:
During our six years of serving seniors in our community and safely maintaining them in their home environment, we have seen a quickly growing need for more information, support, advocacy and counseling for the families and significant others responsible for the care and safety of these aging folks. The health care system has become increasingly complex with ever shifting regulations. Managing resources to effectively care for seniors is critical.
Our experienced health service professionals can help you navigate today’s complex health care systems. Our services include the following:
- Comprehensive evaluation to identify barriers to client’s level of functioning and quality of life in their home setting
- Crisis planning and intervention using our knowledge of
availability of community resources to problem solve
- Customized clinical care planning including medication
management
- Liaison to physicians, family or legal guardians including
attending medical appointments
- Ongoing monitoring and advocacy including review of care plan as needed with changing conditions
- Effective communication with family members or caregivers both locally, nationally and internationally
- Assistance to family members in choosing appropriate residential living facilities, including skilled care, rehabilitation, assistance living and independent living options
Transition Services:
Although we fully realize that most seniors prefer to live in their home environment, there are changes in conditions that lead the family/responsible provider to search for a more structured environment, as an assisted living facility or nursing home. These life changes can be fraught with uncertainty, stress, and potential safety issues for elderly persons. It can also place an unexpected strain on families who are coping with several other issues in their lives.
Our skilled providers offer the following services:
- Assistance in evaluating facilities with the individual needs of the elderly client in mind
- Planning a gradual individualized transition to the chosen facility, placing the family’s and client’s fears to rest
- Helping the seniors become fully acclimated to the facility, the routines, activities, meals, surroundings and fellow residents
- Collaborating with facility staff to insure that the continued care of the clients is comfortable and secure
- Periodically “checking in” with the clients to insure that all their needs are met and any issues quickly resolved to the satisfaction of both the senior and his/her family
